Surgical Consultation – Fee and Cancellation Policy*

Thank you for considering Karolak Facial Plastic Surgeon. The cost of a personalized surgical consultation with Dr. Karolak is $350 when scheduling in order to reserve your time. This fee applies to in-person as well as FaceTime/Skype virtual consults. The fee covers the evaluation, education, planning, and recommendations by Dr. Karolak regarding your aging pattern and facial rejuvenation goals. The $350 may be applied to the cost of surgical procedures only within 12 months of the consultation.

A 48 hour cancellation policy is strictly enforced because of the demand, limited availability, and amount of time reserved in Dr. Karolak’s schedule. Cancellations less than 48 hours will be charged the cost of the consultation fee and forfeit the ability to apply that fee to any future surgery/services. The same policy will apply to rescheduling. Dr. Karolak looks forward to meeting with you!
